If you live within our practice catchment area which covers most of the SW1 postal district, you will need to fill in a Registration form and a New Patient questionnaire to register with the practice. These are available from reception or can be downloaded by clicking the links below. We also welcome patients who live outside the local area but for work or other reasons may wish to join the surgery. This out-of-area registration is not suitable for patients who are housebound or have complex health needs.

Fill out the form
Fill out the form and provide us with two ID forms.
Receive your patient pack
You will be given a patient pack full of relevant information.
Book an Appointment
You will also be asked to book an appointment for a new patient with one of our nurses. Also please would parents bring their child’s Red Book or a list of all immunisations.
Completed forms should be handed in to the practice together with two forms of ID. Proof of address and photographic ID is required.
Please note that we are not able to register children under 16 years old online
Once we have seen your completed documents with the your correct proof of ID, you will be given a new patient pack full of relevant information and asked to book an appointment for a new patient medical with one of our nurses. Do please remember to bring all your current medication with you and any relevant medical details. Also please would parents bring their child’s Red Book or a list of all immunisations.
